How Do I Sign Up For Preschool?
In order to process your child's registration the following forms must be received:
- Physical Examination by a physician is required for all children enrolling in a licensed facility. The form required for this exam is the School Entrance and Physical Examination and Immunization Certification. This form must not be older than two years prior to the enrollment date of the child. This examination MUST be obtained before or within one month of enrollment.
- Evidence that their immunizations are up to date. (The State Board of Health requires immunizations and documentation on the School Entrance and Physical Examination and Immunization Certification. Updated immunizations will be obtained once between a child's fourth and sixth birthday.)
- COMPLETELY filled out Preschool enrollment/registration form.
- Proof of Identity such as a copy of birth certificate.
- A non-refundable annual registration fee is required ($100.00) for preschool.
- All families are required to have an active credit card number on file prior to the start of the program.
- Note: Once a completed enrollment application and registration fee has been received and processed you are considered to be a Kids Club Member. If you should withdraw from, or be terminated from the program for any reason a cancellation fee will apply, regardless of how much notice has been given.

- Proof of Identity such as a copy of birth certificate.
- A two week deposit is required for summer camp. ($260.00 for full time, $170.00 for part time)
- All families are required to have an active credit card number on file prior to the start of the program.
- Note: Once a completed enrollment application has been received and processed you are considered to be a Kids Club Member. If you should withdraw from, or be terminated from the program for any reason a cancellation fee will apply, regardless of how much notice has been given.
